MHS Athletics will be using a new registration platform for the 2023-2024 School Year for collecting student information and health forms. We will still be using MyPaymentsPlus to collect payments. The new registration platform can be access through https://middletoncardinalsathletics.com/ under "Registration."

This is just for collection of the following forms:

• WIAA Physical Forms

• MHS Code of Conduct

• WIAA Concussion and Sudden Cardiac Arrest Form

• Permission to Participate in MHS Athletics and Activities.

  
  

Students must be registered online (completing all forms) and have a current physical or alternate year physical form on file to be able to tryout or practice with the team. 

My PaymentsPlus will still be used to pay fees associated with all athletics and activities.

2023-2024 Transportation Form - Must be completed if a student is being transported in a vehicle that is not a district vehicle. This must be filled out for every instance of "self-transport." Students may not drive themselves, parents/guardians must drive.

Physicals for the 2023-24 school year must be dated 4/1/22 or after to be eligible for the upcoming school year. Please submit all forms through the online registration platform at https://middletoncardinalsathletics.com/

ALTERNATE YEAR FORM is now part of our new registration platform. Go to https://middletoncardinalsathletics.com/ and click on "Registration" to create an account and register your athlete.
If your athlete's physical is between 4/1/22 and 3/31/23, this electronic form must be completed.

Additional ELIGIBILITY forms can be found on the Wisconsin Interscholastic Athletic Association website at www.wiaawi.org.  Click on Schools<Eligibility Rules and Forms. There is pertinent information regarding eligibility and you will find the following forms there:

• EVTS - Eligibility Verification of Transfer Student Form

• Foreign Student Eligibility Verification Form - NOTE:  All Foreign Exchange students must have a U.S. physical.
