To register, please provide the following, either electronically or in person:

1. Proof of Residency (MG&E/Alliant Energy bill or current lease)

2. Proof of student's age (copy of your child's birth certificate)

3. Immunization record (form is included in packet)

4. 4K Registration forms (click to link to open)

Registration forms for Early Childhood and 4K may be filled out electronically. 

If electronic registration does not work for you please print the forms and email them to: jsensenbrenner@mcpasd.k12.wi.us .

Please note:  We are unable to place students in 4K sites if their registration paperwork is incomplete.
