Registration for the MCPASD 4K program for the 2017-18 school year will open on Monday, Jan. 2, 4K Principal Kari Saunders said.
Forms will be available starting on Jan. 2 on the 4K website and families are encouraged to complete the electronic forms available on that site, Saunders said. If families need help completing the forms or have any questions, please call the 4K office at 608-829-2346.
Families can also register in person at the District Services Center Office, 7106 South Ave. in Middleton. Regular office hours are 7:30 a.m. to 4:30 p.m., Monday through Friday.
You will need proof of residency, proof of student's age and immunization record along with a completed 2017-18 4K registration packet to complete the process. The registration packet is also available in Spanish.
Transportation is provided to the morning session of 4K, which is held Monday through Friday from 9 to 11:30 a.m. Transportation is currently not provided to the afternoon session of 4K, which is held Monday through Friday from 12:15 to 2:45 p.m.
The District implemented the 4K program during the 2011-12 school year. More than 1,900 students have participated in the program since then, including more than 330 in 2016-17.
