**MHS WILL NO LONGER ACCEPT PERSONAL CHECKS FOR TRANSCRIPT REQUESTS**

Acceptable forms of payment are cash, cashier's check or money order

Current and former Middleton High School students can obtain a copy of their transcripts by contacting Darcy Middleton in our Student Services - Counseling Department at dmiddleton@mcpasd.k12.wi.us or 608-829-9917.

You will need to submit a Transcript Request Form along with $3.00. A PDF version of the Transcript Request Form is available by clicking here. Please allow 3 - 5 business days for processing your request from the time the Transcript Request Form and the $3 fee is received in our office.

Please note that MHS must have the students or former students signature to release transcripts. Parents may sign for a student under 18 years of age.
