2017-2018 School Year Free Physicals for Uninsured or BadgerCare High School Athletes:

Attention:  All Uninsured High School Athletes

On Tuesday, Aug. 8, 2017 from 6 - 9 p.m. Access Community Health Center, 2202 S Park St., Madison, WI will be offering free sport physicals to uninsured athletes.  

Details:

Last names A-I (arrive @ 6 p.m.)

Last names J - R (arrive at 7 p.m.)

Last names S - Z (arrive no later than 8 p.m.)

In order to receive immunizations, a parent MUST be present.  Please bring immunization records with you.

Female athletes please wear tank tops.

If you cannot attend this clinic, please contact the athletic trainer at your school, Lori Neilitz at 212 2941.

NOTE:  Athletes that begin tryouts and practices prior to August 12th, you will need to acquire your physical elsewhere.  Concentra on Junction Road in Madison does offer physicals for a nominal fee on a walk-in basis.        

 

SIGNING UP FOR  A FALL SPORT VIA FEEPAY:

SIGN-UP FOR A FALL SPORT IS MANDATORY THROUGH FEE PAY AND YOU MAY DO SO AS OF MID JULY, 2017.  THE EXACT DATE IS STILL TBD.    INSTRUCTIONS ON HOW TO USE FEEPAY ARE FOUND VIA THESE LINKS:

 

INCOMING FRESHMAN AND NEW ATHLETE ALL-SPORT MEETING:

There will be a parent and athlete meeting for all incoming freshman and/or new MHS student-athlete interested in playing ANY sport, during any season, for the 2017-18 school year.

The meeting is scheduled for May 30, 2017 from 6 -  7 p.m. in the Performing Arts Center at Middleton High School.  Immediately following that meeting there will be break-out sessions for the fall athletes only.  The designated  locations of those break-out sessions will be provided at 7 p.m.  The winter/spring athletes and their parents may depart at 7 p.m.

Attendance at this meeting is highly encouraged for both parent and athlete.  There will be significant information provided, as well as an overview of the student athletic code.

 

CHEERLEADING TRYOUTS 2017-18 SCHOOL YEAR

Cheer tryouts for the coming school year will be the week of May 29, 2017.  The schedule is as follows:

  • May 30 - Informational meeting for those athletes currently at MHS - 4 p.m. in Room 1701
  • May 30 - Incoming freshman - 6 - 7p.m. in the PAC.  Breakout session following at 7 p.m. in room 1701
  • Tryouts:   May 31, June 1, June 2 -  4:30 - 6:30 p.m. in the MHS Small Gym.
  • Tryouts concluding:  June 5 - 4:30 - 6:30 p.m. in the Concourse (the area between the Field House and Varsity Gym).

Athletic clothing, proper shoes and a water bottle recommended.

 

DANCE TEAM TRYOUTS FOR THE 2017-2018 SCHOOL YEAR:

Dance Team tryouts for the 2017-2018 school year will be the week of May 15th, 2017.  The schedule is as follows:

  • Monday, May 15 - Mandatory Parent and Dancer Meeting - 5:30 p.m. Small Gym
  • Tuesday, May 16 thru Thursday, May 18 - Tryout Clinics - 4 - 6 p.m. in the Varsity Gym and Small Gym
  • Friday, May 19 - Auditions - 4 - 7 p.m. Varsity Gym

2017 FALL SPORT SIGN-UP:

THE FALL SPORT MEETINGS FOR RETURNING MHS ATHLETES WILL TAKE PLACE ON TUESDAY, MAY 30 IMMEDIATELY FOLLOWING SCHOOL IN THE FOLLOWING LOCATIONS:

  • FOOTBALL - SMALL GYM
  • CROSS COUNTRY - FIELD HOUSE NORTH
  • GIRLS GOLF - ROOM 1371
  • BOYS SOCCER - FIELD HOUSE SOUTH
  • GIRLS SWIM/DIVE:  POOL LOBBY
  • GIRLS TENNIS:  ROOM 1704
  • BOYS VOLLEYBALL:  VARSITY GYM EAST
  • GIRLS VOLLEYBALL:  VARSITY GYM WEST
  • CHEER:  ROOM 1701

ATHLETIC FORMS:

All athletic forms, except the physical form, will be submitted electronically when you sign-up to register your student(s) via FeePay.   The forms will auto-populate at the time that you choose the fall sport or any year long club that charges a club user fee. 

For those that do not have access to a computer, the athletic office will accept hardcopy forms.  We anticipate 90% of all required forms, EXCEPT THE PHYSICAL FORM, to be submitted electronically.  

NOTE:  ALL PHYSICAL FORMS MUST BE TURNED INTO THE ATHLETIC OFFICE PRIOR TO THE FIRST DAY OF TRYOUTS/PRACTICE OR  YOUR STUDENT WILL BE INELIGIBLE TO PARTICIPATE UNTIL A VALID DATE IS ON FILE.  

You will be unable to register for a winter or spring sport via FeePay until a pre-determined date established by the district.  A district correspondence will go out in October and March informing you that you are able to sign up via FeePay.  Again, at the time you choose the sport you wish to sign your student up for and the forms needed will then auto-populate.  These forms include the consent form, concussion form, emergency contact card and transportation/driving waivers.  Again, the only form that you will need to turn into the athletic office will be the MEDICAL CLEARANCE FORM which you can now find here on the MHS website under Athletics<Athletic Information<Forms.
