Retail Items Permission Form
Permission is required for students to charge extra food or milk with the Lunch Express program. This permission form will carry over from year to year. download form

Lunch Express Withdrawal Form
This form needs to be filled out when you no longer have any children enrolled in the Middleton-Cross Plains Area School District. It allows you to receive the balance of the funds left in your Lunch Express account or to donate the funds left to the Student Meal Donation Fund to help needy families pay for their outstanding Lunch Express balances. Filling out this form will also tell us to delete your family from our files and our calling system. download form
Student Meal Donation Fund Request Form
Request assistance in paying for outstanding student meal bills and insure that all students are provided a nutritious meal. Requests for assistance can be made by parents, Lunch Express personnel or school personnel.download form (doc)

Current Year's Newsletters
Learn about the School Nutrition program through the current year's news for kindergarten, elementary, middle school and high school students and parents.
Each newsletter contains information about who contact if you have questions, our District's nutrition philosophy, frequently asked questions, an explanation about our Lunch Express debit system and a permission slip for those students eligible for retail (a la carte) purchases.
